Questions About This Form

Is this form applicable to Family Law, and, as there is no request for documentation in support of having someone declared vexatious, is there another form(s) to initiate the process? Can this form be included with an upcoming RFO hearing as long as the forms are filed correctly and served on the other party prior to the hearing? What if I am unable to access other family law cases which concern the vexatious litigant, but are needed to prove the threshold of number of cases to be declared vexatious? Thank you!
